Vision & Strategy
We as a Primary Care Network (PCN) will look at our overall population needs and health inequalities between member practices and work together to provide care at greater scale. By sharing staff we will address these differences and also provide a standardised care for other long-term conditions. We aim to improve our patients experience and make better use of technology to support their health needs.
Values
Services will be developed locally, with the aim of providing a proactive, personalised and coordinated response, based on the needs of the patients that we serve. As an example, we have introduced new roles such as a social prescriber to enable GPs to refer patients to a link worker who will support them to access help provided by community services to combat loneliness and social isolation.
Ambition
Our overall aim is to close the gap between general practice and other Integrated Care Systems and provide a safe, effective and good quality GP service now and in the future.
We as a PCN, eventually, hope to be able to integrate primary care with secondary and community services.
We hope to be able to standardise care between our member practices and address any inequalities of health and care.
